Urticaria şi angioedemul induse de stres

Pielea este unică din multe puncte de vedere, fiind cel mai mare organ al nostru și reprezentând prima linie de apărare a organismului în interacțiunea cu diverse elemente din mediu.
Stresul apare ca adaptare la răspunsul “fight or flight” (luptă sau fugi) secundar diverselor circumstanțe încărcate psiho-emoțional. Semnalele de stres acționează la nivelul axului hipotalamo-hipofizar și a sistemului nervos simpatic rezultând o descarcare de hormoni și mediatori ce pot induce sau declanșa exacerbări ale afecțiunilor cutanate ce au la bază un mecanism mediat imun. Urticaria reprezintă o patologie cutaneo-mucoasă frecventă cu debut brusc prin apariția de papule eritematoase de diferite dimensiuni, pruriginoase, edematoase, uneori dureroase sau dând senzația de arsură, ce se remit spontan (caracter fugace) în mai puțin de 24 de ore și pot să reapară în altă zonă decât cea inițială, fiind insoțite sau nu de angioedem. În timp ce urticaria acută se remite în mai puțin de 6 săptămâni, urticaria cronică (>6 saptamani) conduce la o alterare semnificativă a calității vieții atât pe plan social cât și professional. Un aspect important în diagnosticarea corectă este discriminarea între urticarial afecțiune și urticaria simptom ce acompaniază bolile infecțioase, patologiile autoimune, anafilaxia, angioedemul ereditar, patologiile oncologice, reacțiile secundare testelor cutanate, sindroamele de activare mastocitară etc.

The skin is unique in many ways, being our largest organ and representing the first line of defense of the body in interacting with various elements in the environment.

Stress appears as an adaptation to the “fight or flight” response that occurs in the various psycho-emotional incidents of everyday life. Stress signals act on the hypothalamic-pituitary axis and sympathetic nervous system resulting in a discharge of hormones and mediators that can induce or trigger exacerbations of cutaneous conditions that are based on an immune-mediated mechanism.
Urticaria is a frequent cutaneous-mucosal pathology with sudden onset of erythematous wheals of varying sizes, pruritic, edematous, sometimes painful or burning sensations that spontaneously emerge in less than 24 hours and can reappear in a different area than the initial one, with or without angioedema.
While acute urticaria resolves in less than 6 weeks, chronic urticaria (> 6 weeks) leads to a significant alteration in the quality of life both socially and professionally. An important aspect in correct diagnosis is the discrimination between urticaria as a condition and the urticaria symptom accompanying infectious diseases,autoimmune pathologies, anaphylaxis, hereditary angioedema, oncological pathologies, skin test reactions, mast cell activation syndromes, etc.
